UCARE students will assist researchers with our funded self-management intervention to address the challenge of adherence to chronic disease clinical treatment guidelines. Our 8-week self-management intervention uses tailored strategies (problem-solving, decision-making, action planning, self-tailoring, resource utilization and forming a patient-provider partnership) and a mobile health app. Patients enrolled must be diagnosed with heart failure, COPD, diabetes or hypertension and have at least one other chronic disease to participate in the study. This feasibility study will evaluate the impact of delivering a mobile health self-management intervention in the multimorbid population returning home after hospital discharge.
